Patient Bill of Rights

About this policy

As a patient of Ongo Weight Loss, you have the right to receive care that respects your dignity, autonomy, and wellbeing. This Patient Bill of Rights describes what you can expect from us and from the independent, licensed Providers available through our platform. It supplements, and does not replace, our Consent to Telehealth Services, HIPAA Notice of Privacy Practices, and Privacy Policy.

Respectful and Non-Discriminatory Care

Receive care that is considerate and respectful of your dignity, without discrimination on the basis of race, color, national origin, religion, sex, gender identity, sexual orientation, age, disability, or any other characteristic protected by law.

Be Informed

Receive clear, accurate information about your diagnosis, treatment options, and the medications available to you, including whether a medication is FDA-approved or compounded, in language you can understand, so you can make informed decisions about your care.

Participate in Your Treatment Decisions

Participate in decisions about your care and ask questions about any recommended treatment, including its risks, benefits, and alternatives, before agreeing to proceed.

Know Who Is Treating You

Know the identity, professional credentials, and licensure of the Provider treating you, including their NPI registration and the state(s) in which they are licensed to practice.

Refuse Treatment

Decline any recommended treatment or medication, or discontinue care at any time, without it affecting your ability to seek future care through Ongo or elsewhere.

Privacy and Confidentiality

Have your health information kept private and confidential, and used and disclosed only as described in our HIPAA Notice of Privacy Practices, our Consumer Health Data Privacy Notice, and applicable law.

Access Your Medical Records

Request and receive a copy of your medical records, and request that we send your treatment information to your primary care physician, as described in our HIPAA Notice of Privacy Practices.

Understand the Cost of Your Care

Receive clear information about the cost of your consultation and any medication before you are charged, including whether you are enrolling in a one-time service or a recurring subscription.

Receive Care Free from Retaliation

Voice a concern, complaint, or question about your care without fear that doing so will affect the quality of care you receive.

Understand the Limitations of Telehealth

Be informed that telehealth has limitations compared to an in-person visit, and that your Provider may determine that telehealth is not appropriate for your condition and may recommend in-person or emergency care instead, as described in our Consent to Telehealth Services.

Understand Compounded Medications

Receive clear disclosure when a prescribed medication is a compounded product rather than an FDA-approved medication, including that compounded drugs are not reviewed or approved by the FDA for safety, effectiveness, or quality, so you can discuss whether a compounded medication is appropriate for you with your Provider.

File a Complaint

File a complaint about your care or your Provider without fear of retaliation. You may contact us directly at info@ongoweightloss.com or 1-888-655-5267, or you may contact the medical board of the state in which your Provider is licensed.

Emergency Care

Understand that Ongo's telehealth services are not intended for medical emergencies. In a medical emergency, call 911. If you are experiencing a mental health crisis, call or text 988 (Suicide & Crisis Lifeline).

Your Responsibilities as a Patient

To help us provide you with safe and effective care, we ask that you:

  • Provide accurate and complete information about your health history, current medications, and symptoms
  • Follow the treatment plan you and your Provider agree upon, or discuss any concerns with your Provider before discontinuing it
  • Notify your Provider promptly of any side effects or changes in your condition
  • Be truthful about your location during telehealth visits, consistent with our Consent to Telehealth Services
  • Treat our staff and Providers with courtesy and respect
